Carle Foundation Physicians - Mattoon/Charleston offers a wide range of services including primary care, specialty care and many other support services, such as imaging/x-ray services. Patients have access to exceptional full-time physicians who practice in a variety of areas and numerous visiting physician specialists who have regular office hours at the Mattoon/Charleston location.

Other services available at the clinic include a registered dietitian, daytime, evening and weekend Convenient Care walk-in hours and women's health services.

As part of our effort to fully integrate all processes across the Carle system, we will be implementing a new billing system on January 1, 2011, and will need to verify your billing information. Please arrive 15 minutes early and bring your insurance card to your next appointment.

All professional or outpatient services you receive after January 1, 2011, will be billed from Carle Foundation Physicians, PO Box 6004, Urbana, IL 61803-6004. This bill will be green and white, and feature a Carle Foundation Physicians logo.  

If you have a balance for services provided before January 1, 2011, you will continue to receive a bill generated by our old system, with a payment of address of Carle Foundation Physician Services, PO Box 911287, Dallas, TX 75391-1287. This bill is red and white. Online bill pay remains available for these balances.
